Contribution Form Summary


Key To The City's information contribution form is used to gather information to present on Key To The City's City-Pages. The information gathered by this form will be used in the Community News section of the City-Page. The displayed content, or entry, will consist of information about community news in the area followed by a reference citation.  The FIRST form page asks for the type of source document used(newspaper, book, etc.).  The SECOND form page asks for more source information(author, Publisher, etc.). This source information will be used to create the citation portion of the City-Page section entry.  The THIRD form page asks for the community news description portion of the entry.

This form page collects citation information for the Community News section of , where the source type is Other. It has six input fields, four of which are required.

USAGE

The Other source type is used when other source types are not available in the form. There are four required input fields, however because of the uniqueness of some unusual source types all 4 required input fields may not apply. For example a radio ad, or personal interview. When an input field is not applicable put "na" in the field.

While some entries might be considered common knowledge and not require a citation in most circumstances, please use the Other contribution form source type to provide at least some citation information. Even if the information is not adequate on its face but it is of significant interest it could lead to additional sources that are valuable.

The "Other" source type input fields have the same purpose as source type fields such as book, newspaper, magazine etc. The names of the fields are the same also. The difference is that it is more difficult to identify what information goes where. Here are some explanations that will help you decide which fields are applicable for the citation information you have.

"Author" input field:

For our purposes the author of the source is the creator of the information displayed on our City-Page sections. An interviewee or wittness of an event or activity would be the author. Similarly the creator of a report or other record of information would be the author. If the author is unknown leave this field blank.

"Name Of Source" input field:

Besides typical sources like a book or newspaper; a chart, report, interviewee, wittness and more can also be source names. You can think of it as a description of the source as well as it's name. If more than one or two words are needed, use the description input field instead of the name field. In which case enter "na" in the name field.

"Publisher Name" input field:

Providing a publisher name adds credibility to the source. Besides adding credibility, knowing the publisher makes it easier to use the source to find other information. If a work is self published or has no publisher, the author and the publisher will be the same.

"Source Date" input field:

This is the date of publication, or if the work is unpublished the creation date. If the date is not available leave this field blank.

"Source Description" input field:

Use this input field to described the source if needed, otherwise input "na". This field is useful if the source hasn't a "name", or the name is ambiguous.

"Page Number input" field:

If there is no page number input "na".

- To return to the "active" City-Page click on the blue link at the lower left.
- To go to page 3 of the contribution form click "Next".
- To clear all of the input boxes click "Reset".
